Emergency management in severe hypothermia - case study

Wstęp. Organizm ludzki bezustannie narażony jest na deficyt energii cieplnej, nawet w warunkach, w których odizolowany jest od środowiska zewnętrznego. W momencie gdy mechanizmy termoregulacji nie są w stanie utrzymać temperatury narządów wewnętrznych w fizjologicznej wartości, dochodzi do hipotermii skutkującej kaskadą zaburzeń narządowych i układowych. W praktyce ratownika medycznego hipotermia może stanowić wyzwanie diagnostyczne jak i terapeutyczne ze względu na złożony obraz kliniczny oraz zmodyfikowane wytyczne w postępowaniu. Leczenie pacjenta z hipotermią powinno być jednocześnie zdecydowane i ostrożne. W pracy poruszono problem wychłodzenia wśród osób bezdomnych. Cel pracy. Celem pracy było przedstawienie opisu postępowania ratowniczego w przypadku poszkodowanego z zatrzyamniem krążenia w ciężkiej hipotermii.Materiały i metody. W pracy wykorzystano metodę studium przypadku.Wnioski. Przedstawiony opis przypadku pozwala zwrócić uwagę na konieczność stosowania odmiennych decyzji terapeutycznych w stosunku do zatrzymania krążenia z powodu innych przyczyn oraz podkreślić priorytety działań ratowniczych.

Introduction. The human body is constantly exposed to a deficit of thermal energy, even when it is isolated from the external environment. When thermoregulation mechanisms are unable to maintain the temperature of internal organs at the physiological level, hypothermia develops, resulting in organ and systemic disorders. In the paramedic practice, hypothermia may be a diagnostic and therapeutic challenge due to the clinical picture and modified guidelines. Treatment of a hypothermic patient should be determined and careful at the same time. The work focuses on the problem of hypothermia among the homeless.Purpose of research. The aim of the study was to present a rescue procedure in a severely hypothermic victim with cardiac arrest.Materials and methods. The case study method was used in the work.Conclusions. The presented case report highlights the necessity of different therapeutic decisions in relation to cardiac arrest and emphasizes the priorities of rescue actions.
