Relations between the feeling of influence on the course of disease and the management with diabetes

WSTĘP. Poczucie wpływu na przebieg choroby jest istotnym czynnikiem psychologicznym, wpływającym na wypełnianie przez chorych zaleceń lekarskich, a także na preferowane przez nich metody leczenia. Celem badania była ocena tego typu związków. MATERIAŁ I METODY. W badaniu wzięło udział 1199 chorych - 655 kobiet (54,6%) i 544 mężczyzn (45,4%), w wieku 19-93 lat (średnia wieku 62,23 roku; SD = 11,08), leczonych z powodu cukrzycy przez okres od kilku miesięcy do 36 lat (średnio 8,03 roku; SD = 5,54). Do oceny poczucia wpływu na przebieg choroby zastosowano Krótką Metodę Oceny Poczucia Wpływu na Przebieg Choroby. WYNIKI. W grupach chorych bez odpowiedniej wiedzy na temat cukrzycy oraz z problemami w zakresie radzenia sobie ze stresem średnia wartość poczucia wpływu na przebieg choroby była istotnie niższa. Poczucie wpływu na przebieg choroby ma rosnący związek ze skutecznością leczenia, ocenianą na podstawie oznaczeń stężenia HbA1c. W grupie osób stosujących najprostszy, duży wstrzykiwacz średni poziom nasilenia poczucia wpływu na przebieg choroby był istotnie niższy od poziomu, który występował w grupach stosujących mniejsze i bardziej skomplikowane urządzenia. WNIOSKI. Poczucie wpływu na przebieg choroby wiąże się z wyborem metod leczenia i stosowaniem się do zaleceń lekarskich.

INTRODUCTION. The feeling of influence on the course of disease is a meaningful psychological factor related with patients adherence to treatment and also the choice of the methods of treatment. The assessment of these relations is the goal of the study. MATERIAL AND METHODS. 1199 patients [655 females (54,6%) and 544 males (45,4%)], of age 19-93 years (M = 62,23; SD = 11,08). They were beginning insulinotherapy after a period of therapy ranging from several months to 36 years (M = 8,03; SD = 5,54). The Brief Scale For the Feeling of an Influence on the Course of Disease Assessment: Version for Diabetes was applied. RESULTS. The mean score of the feeling of an influence on the course of disease was statistically significantly lower in the groups of patients without adequate knowledge about diabetes and in the group of patients with problems with the stress management. The feeling of an influence on the course of disease has an increasing relation with the effectiveness of the treatment assessed with HbA1c level, together with the duration of the disease. In the group with the choice of the simplest disposable injector, the mean level of the feeling of an influence on the course of disease was statistically significantly lower than in groups that chose smaller and more complicated types of injectors. CONCLUSION. The feeling of an influence on the course of disease is related with the choice of methods of treatment and with patients adherence to the treatment.
