The model of care for a patient with Lyme disease based on the nursing theory of D. Orem

Wprowadzenie: Borelioza z Lyme jest wielonarządową, bakteryjną chorobą zakaźną przenoszoną przez kleszcze z rodzaju Ixodes, wywołana krętkami z rodzaju Borrelia. Choroba przebiega w trzech stadiach z zajęciem serca, stawów, skóry oraz układu nerwowego. Diagnozę stawia się na podstawie obrazu klinicznego potwierdzonego badaniami serologicznymi. Nieleczona lub niereagująca na leczenie borelioza prowadzi do pogorszenia jakości życia pacjenta. Działania podjęte przez pielęgniarkę powinny więc zmierzać do jej poprawy przez zwiększenie zakresu samoopieki pacjenta. Modelem pielęgnowania, który może przygotować pacjenta do samoopieki jest model Dorothy Orem. Cel pracy: Celem pracy było opracowanie modelu opieki nad pacjentem z boreliozą w oparciu o teorię pielęgnowania Dorothy Orem i klasyfikację diagnoz pielęgniarskich według NANDA.Materiał i metody: W pracy zastosowano metodę analizy naukowego piśmiennictwa medycznego.Wyniki: Najważniejszą diagnozą pielęgniarską u pacjenta z boreliozą jest zaburzenie pracy serca i przewodnictwa, wynikające z zajęcia mięśnia sercowego przez bakterie boreliozy. Z kolei główną dolegliwością jest ból, występujący we wszystkich stadiach choroby, mogący pochodzić z każdego zajętego układu lub narządu. Do najważniejszych interwencji pielęgniarskich należy udział w procesie diagnozowania i leczenia oraz przygotowanie pacjenta do samoopieki.Wnioski: Model opieki Dorothy Orem ukazuje aktualne i potencjalne problemy zdrowotne pacjenta z boreliozą oraz interwencje pielęgniarskie, których celem jest przygotowanie pacjenta do samoopieki przez wyeliminowanie deficytów.

Introduction: Lyme disease is a multiorgan, bacterial infectious disease transmitted by ticks of the Ixodes genus, caused by spirochetes of the Borrelia genus. The disease proceeds in three stages with involvement of the heart, joints, skin and nervous system. Diagnosis is based on the clinical picture confirmed by serological tests. Untreated or nonreactive for the treatment Lyme disease leads to deterioration in the quality of patient’s life. Therefore, measures taken by the nurse should tend to improve it by increasing the range of patient’s self-care. Nursing model, which can preparing the patient for self-care is a model of Dorothea Orem.Aim: The aim of this study was to elaborate a model of care for a patient with Lyme disease based on the nursing theory of Dorothea Orem and classification of nurse diagnoses by NANDA.Materials and methods: Methods used for the study based on analysis of scientific medical literature.Results: The most important nursing diagnosis of a patient with Lyme disease is a disorder of the heart rate and conduction, resulting from involvement of the heart by Lyme disease bacteria. However, the main disorder is pain that occurs in all stages of the disease, which could be from any morbid organ or system. Participating in diagnosis, treatment and preparing patient for self-care are the most important nursing interventions.Applications: The Dorothy Orem’s model of care shows the current and potential health problems of a patient with Lyme disease. It also presents appropriate nurse interventions in order to prepare the patient for self-care by eliminating deficits.
