Imaging diagnostics of rheumatoid arthritis in a 67-year-old patient - a case study

Reumatoidalne zapalenie stawów (RZS) jest chorobą autoimmunizacyjną. W przebiegu tego schorzenia najczęściej zajęte są stawy śródręczno-międzypaliczkowe, nadgarstkowe i międzypaliczkowe bliższe. Pojawiają się także powikłania, które prowadzą do inwalidztwa. Dlatego istotna jest szybka diagnostyka i ocena radiologiczna tej choroby w celu wdrożenia leczenia i zapobiegania komplikacjom. Pierwsza część pracy zawiera zagadnienia ogólne dotyczące budowy anatomicznej układu szkieletowego i stawu ramiennego, etiologii, epidemiologii, objawów klinicznych, klasyfikacji oraz najczęstszych powikłań reumatoidalnego zapalenia stawów. Przedstawiono diagnostykę laboratoryjną oraz diagnostykę obrazową RZS z uwzględnieniem badania rentgenowskiego (RTG), tomografii komputerowej (TK), rezonansu magnetycznego (MR) oraz badania ultrasonograficznego (USG).W drugiej części pracy przedstawiono diagnostykę obrazową reumatoidalnego zapalenia stawów na podstawie przypadku pacjentki z zapaleniem stawu ramiennego. W przypadku obrazowania zmian w reumatoidalnym zapaleniu stawów badaniem z wyboru jest RTG. W celu poszerzenia diagnostyki wykonuje się badanie MR, USG oraz TK.

Rheumatoid arthritis (RA) is an autoimmune disease, in which usually wrist, metacarpophalangeal and interphalangeal joints are attacked. There are also the complications that may lead to invalidity. It is important to carry out rapid diagnosis and radiological evaluation of this disease in order to implement the treatment and prevent the complications.The first part of this dissertation contains general information about the anatomy of the skeleton and shoulder, as well as the aetiology, epidemiology, clinical symptoms, classification and possible complications of rheumatoid arthritis. This is followed by laboratory diagnostics and imaging diagnostics of the disease, including radiography (RTG), computed tomography (CT), magnetic resonance imaging (MRI) and ultrasonography (USG).The second part of this work describes the imaging diagnostics of a clinical case of the shoulder joint inflammation, with particular attention to imaging diagnostics that was carried out. In the case of imaging diagnostics of changes in rheumatoid arthritis, RTG is the most commonly performed examination. In order to broaden the diagnostics, MRI, USG and CT may be carried out.
