The rehabilitation of the patient with pulmonary heart disease - a case study.

W przebiegu chorób układu oddechowego, w których dochodzi do zwiększenia ciśnienia płucnego, dość częstym powikłaniem u pacjentów jest przewlekłe serce płucne (PSP). Występowanie nadciśnienia płucnego i PSP prowadzi do wielu powikłań, w których postępowanie fizjoterapeutyczne jest jedną z głównych metod leczenia. Kompleksowa i prawidłowa rehabilitacja przynosi znakomite efekty w zmniejszaniu duszności, męczliwości, sinicy centralnej czy obrzęków. Fizjoterapia pacjentów z przewlekłym sercem płucnym skupia się głównie na obniżeniu nadciśnienia płucnego, a tym samym ograniczeniu postępu patologii w obrębie prawej komory serca. Celem pracy jest przedstawienie postępowania fizjoterapeutycznego oraz metod wykorzystywanych w trakcie rehabilitacji pacjenta z PSP. Praca ma charakter kazuistyczny. W pierwszej części pracy została przedstawiona etiopatogeneza przewlekłego serca płucnego, diagnostyka i leczenie, a także metody rehabilitacji. Druga część zawiera opis przypadku 70- letniego mężczyzny z rozpoznanym PSP powstałym w wyniku wieloletniego przebiegu przewlekłej obturacyjnej choroby płuc (POChP), u którego zastosowano kompleksową terapię krążeniowo-oddechową. Proces rehabilitacyjny opierając się głównie na rehabilitacji oddechowej oraz ćwiczeniach ogólnowydolnościowych, poprawia kondycję pacjenta, jego samopoczucie, jakość życia i stanowi obok farmakoterapii niezwykle istotny element leczenia.

During the course of pulmonary diseases in which the pulmonary preassure is elevated pulmonary heart disease is quite common complication. The presence of pulmonary hypertension and pulmonary heart disease leads to many disabilities in which the most important treatment is physiotherapy. Comperhensive and proper rehabilitation brings excellent results in reducing dyspnoea, fatigue, central cyanosis and swelling. Physiotherapy among patients with pulmonary heart disease focus mainly on reducing pulmonary hypertension and also restrict access of pathology in right ventricle. The aim of this paper is the presentation of rehabilitation and diffrent methods that are used during physical treatment. The first part of this work contains the etiology, pathophysiology of pulmonary heart disease. The second part includes description of a 70 years old patient with this illnes which developed as a result of chronic obstructive pulmonary disease. The patient was treated by complex cardiorespiratory therapy. This part of work includes diagnostic methods used during rehabilitation, their descriptions and the ways of action. The process of rehabilitation is based on pulmonary rehabilitation and cardiovascular exercises that improves exercise capacity and quality of life. Rehabilitation is very important in pulmonary heart disesase. It is an essential element of treatment together with pharmacological treatment.
