The nurse's role in preparing for self-care patients with rheumatoid arthritis - a case study

Wstęp: Reumatoidalne zapalenie stawów (RZS) to przewlekła choroba zapalna błony maziowej małych i średnich stawów, prowadząca do uszkodzenia chrząstek stawowych, kości i innych struktur tworzących stawy. Ma charakter zapalny, przewlekły, ogólnoustrojowy oraz autoimmunologiczne podłoże. Reumatoidalne zapalenie stawów w początkowym etapie rozwoju powoduje destrukcję stawów, ograniczenie sprawności fizycznej, które stopniowo postępują w czasie, a także znacznie obniżają jakość życia. Niniejsza praca w pierwszej części przedstawia kliniczny opis reumatoidalnego zapalenia stawów (definicja, rys historyczny, epidemiologia, etiopatogeneza, badania diagnostyczne, obraz kliniczny, leczenie, rokowanie), natomiast w drugiej części opracowano proces pielęgnowania, przedstawiający problemy zdrowotne występujące u wybranej pacjentki. Podjęto również temat roli pielęgniarki w przygotowaniu pacjentki z RZS do samoopieki.Cel pracy: zaplanowanie indywidualnego planu opieki dla pacjentki z reumatoidalnym zapaleniem stawów w oparciu o analizę stanu biopsychospołecznego; przedstawienie roli pielęgniarki w przygotowaniu chorej do samoopieki i samopielęgnacji. Materiał i metody: pracę przygotowano w oparciu o metodę studium przypadku oraz analizę dostępnego piśmiennictwa; praca ma charakter kazuistyczny. Wyniki: Diagnozy pielęgniarskie postawione w wyniku choroby reumatoidalnego zapalenia stawów bazowały na wymiarze: fizycznym, psychicznym oraz społecznym. Dotyczyły głównie dolegliwości bólowych, ograniczeń w podejmowaniu aktywności fizycznej i społecznej oraz radzenia sobie z podstawowymi czynnościami dnia codziennego. Istotną rolę w opiece nad pacjentką z RZS stanowiło przygotowanie chorej do samoopieki i samopielęgnacji.Wnioski: metoda studium przypadku pozwoliła na opracowanie indywidualnego procesu pielęgnowania, który uwzględnił ocenę sytuacji zdrowotnej pacjentki na reumatoidalne zapalenie stawów i jej potrzeb pielęgnacyjnych. Analiza piśmiennictwa dała możliwość poszerzenia wiedzy na temat wyżej wymienionej jednostki chorobowej. Słowa kluczowe: reumatoidalne zapalenie stawów, RZS, samoopieka, proces pielęgnowania

Introduction: Rheumatoid arthritis (RA) is a chronic inflammatory disease of the synovial joints of small and medium-sized, leading to damage to the articular cartilage, bones and other means of establishing joints. It has the character inflammation, chronic, systemic and autoimmune background. Rheumatoid arthritis in the initial stage causes destruction of the joints, reducing fitness, which is progressing steadily in time, and also significantly reduce the quality of life. This paper in the first part presents the clinical description of rheumatoid arthritis (definition, historical background, epidemiology, pathogenesis, diagnostic, clinical, treatment, prognosis), in the second part of the nurturing process was developed, presenting health problems occurring in selected patients. Also taken on the role of nurses in preparing a patient with RA to self-care.The aim of the study: To schedule an individual care plan for a patient with rheumatoid arthritis based on an analysis of the biopsychosocial state.Material and Methods: work prepared using the method of case study and analysis of the available literature; work is a casuistic.Results: Diagnosis Nursing posed by the disease of rheumatoid arthritis were based on the dimensions: physical, mental and social aspects. Mainly related to pain, limitations in making physical activity and social and dealing with basic activities of daily living. An important role in the care of the patient with RA accounted for preparing the patient for self-care.Conclusions: The case study method allowed the development of the individual nursing process, which upheld an assessment of the health situation of patients with rheumatoid arthritis and its needs care. The analysis of literature gave the opportunity to expand their knowledge on the above-mentioned disease entity.Keywords: rheumatoid arthritis, RA, self-care, nursing process
