Model of care for patient in a coma during the course of liver cirrhosis

Encefalopatia wątrobowa stanowi jeden z najpoważniejszych powikłań marskości wątroby. Mechanizm jej powstania związany jest przede wszystkim z zaburzoną funkcją detoksykacyjną wątroby, polega na szkodliwym działaniu toksycznych związków, a w głównej mierze amoniaku na układ nerwowy. Nieleczona w początkowym stadium może doprowadzić do rozwoju śpiączki. Chory w śpiączce wątrobowej wymaga kompleksowej opieki na oddziale intensywnej terapii, zabezpieczenia podstawowych funkcji życiowych i przeciwdziałanie powikłaniom ze strony poszczególnych narządów. Celem mojej pracy było przedstawienie przykładowego modelu opieki nad chorym w śpiączce wątrobowej w przebiegu marskości wątroby. Zastosowana przeze mnie struktura jaką był proces pielęgnowania pozwoliła na holistyczną ocenę stanu pacjenta. Zebrane informacje na temat etiologii, objawów i leczenia choroby pozwoliły na sformułowanie diagnoz pielęgniarskich i ustalenie planu opieki. U pacjentów nieprzytomnych z powodu niemożności kontrolowania przez nich pracy poszczególnych narządów wymagane jest podjęcie odpowiednich działań pielęgnacyjnych w kierunku wszystkich prezentowanych objawów jak również możliwych powikłań. W pracy opisano 12 problemów zdrowotnych, określono cel, plan opieki oraz oczekiwane efekty postępowania leczniczo- pielęgnacyjnego.

Hepatic encephalopathy is one of the most serious complications of liver cirrhosis. The mechanism of its formation is primarily associated with the impaired function of the liver detoxification, the harmful effect of toxic compounds on the nervous system, mainly ammonia. Untreated in the early stages it may lead to a coma. Patients in a hepatic coma requires complex care in the intensive care unit, protection of vital functions, and prevention from any complications of individual organs. The aim of my work was to present an example of patient care in a hepatic coma during the course of liver cirrhosis. The structure I presented was the process of patient care that allowed for a holistic assessment of the patient’s condition. The collected information about the etiology, the symptoms and the treatment of the disease allowed formulating a nursing diagnoses and determining a plan of patient care. At unconscious patients, who cannot control the function of their organs, make it necessary to undertake adequate patient care toward all the presented and necessary complications. This document describes 12 health problems, defines the purpose, plan of patient care, and expected effects of the medical treatment and nursing care.
