Highly specialized services the field of organ transplantation funded by the Ministry of Health - the analysis of change

W poniższej pracy podjęto próbę analizy problematyki związanej z realizacją świadczeń wysokospecjalistycznych w Polsce w ostatnim dziesięcioleciu, w ujęciu ogólnym oraz w rozbiciu na poszczególne województwa. Głównym celem pracy była analiza zmian, jakie zaszły w medycynie transplantacyjnej, z uwzględnieniem zagadnień związanych z finansowaniem tych świadczeń przez Ministerstwo Zdrowia. W pracy zostały zebrane i omówione podstawowe wskaźniki stosowane w transplantologii. Korzystano głównie z biuletynów informacyjnych wydawanych przez Centrum Organizacyjno – Koordynacyjne Do Spraw Transplantacji Poltransplant, danych finansowych udostępnionych przez Ministerstwo Zdrowia, raportów GUS o ochronie zdrowia, oraz ustaw z zakresu świadczeń opieki zdrowotnej. Przeprowadzono także przegląd literatury.W pierwszym rozdziale, została opisana instytucja jaką jest Ministerstwo Zdrowia. Omówiono akty regulujące procedury przeszczepiania narządów. Dokonano charakterystyki katalogu świadczeń wysokospecjalistycznych, oraz sposób wyboru realizatorów świadczeń przeszczepowych.W rozdziale drugim, przeprowadzono porównanie sytuacji w transplantacji narządów pomiędzy krajami Unii Europejskiej w roku 2008, wykorzystując do tego wybrane wskaźniki dotyczące transplantacji. W kolejnym rozdziale zostały przedstawione dane, które charakteryzują sytuację w polskiej transplantologii, zwracając uwagę na liczbę dawców, liczbę pobrań i przeszczepień w stosunku do potrzeb.Rozdział czwarty zawiera porównanie sytuacji w liczbie zgłaszanych dawców jak i wykonywanych przeszczepień narządów pomiędzy województwami, przy pomocy wskaźnika liczby pobrań narządów w przeliczeniu na milion mieszkańców.W rozdziale piątym, przedstawiono nakłady finansowe na poszczególne procedury przeszczepowe w ujęciu nominalnym jak i realnym w ciągu roku, oraz wydatki na pojedyncze świadczenie przeszczepowe. Pokazano udział wydatków pochodzących z budżetu państwa na świadczenia wysokospecjalistyczne w całości wydatków budżetowych na ochronę zdrowia W ostaniej części pracy, zaprezentowano wnioski z przeprowadzonej analizy.

The following study presents an analysis of issues related to the implementation of highly specialized services in Poland in the last decade, in general approach as well as in each regions. The main objective of this study is the analysis of the changes that have occurred in transplant medicine, including issues related to the financing of these services by the Ministry of Health.Moreover, the basic indicators that are used in transplantation have been collected and discussed in the following paper.The main source of information was taken from bulletins published by the Centrum Organizacyjno – Koordynacyjne Do Spraw Transplantacji Poltransplant as well as from financial data provided by the Ministry of Health, the specialist literature, the GUS reports of health care, and the acts of healthcare services.The first chapter presents a detailed description of the institution of the Ministry of Health, the characteristic of the catalogue of the highly specialized services, and the way of choosing transplant providers. In the second chapter, there is a comparison between the situation of the transplantation of organs between countries of the European Union. In order to facilitate this comparison some indicators of organs transplantation have been used.The subsequent chapter provides some data that characterize the situation of transplantation in Poland, such as the number of donors, organ donation and transplantation in relation to actual needs.The next chapter presents a comparison of the situation of transplants between all of the Polish regions. Using the number of organ donor per million of population indicated the activity in organ transplantation in particular regions. The last chapter contains the financial expenditure for each transplant procedure in nominal and real terms during the year, as well as the costs of a single transplant service. Finally, the study shows the percentage of expenditure on services associated with the organs transplantation in relation to the overall state spending on health care in Poland.The conclusions of the analysis are presented in the last part of this MA thesis.
