Psychological preparation to the knee replacement surgery and its impact to the post operational period

Praca poświęcona jest roli komunikacji terapeutycznej w przygotowaniu psychicznym pacjentów do poważnych zabiegów ortopedycznych. Celem badań była ocena wpływu przygotowania psychicznego pacjentów, u których wykonano zabieg endoprotezoplastyki stawu kolanowego, na przebieg okresu pooperacyjnego. W badaniu brało udział 60 pacjentów oddziału ortopedyczno-urazowego, z których 30 osób zakwalifikowano do grupy eksperymentalnej i 30 do grupy kontrolnej. Grupę kontrolną przygotowywano do zabiegu operacyjnego w sposób standardowy, podczas gdy w grupie eksperymentalnej pielęgniarka przeprowadziła dodatkową rozmowęo charakterze psychologiczno-wspierającym. W grupie eksperymentalnej uzyskano wyraźną poprawę przebiegu okresu pooperacyjnego w porównaniu z grupą kontrolną. Wykazano różnice w zakresie szybkości uzyskania sprawności i samodzielności, w zużyciu mniejszej ilości dawek leków nasennych i przeciwbólowych, w samoocenie okresu pooperacyjnego, w lepszym samopoczuciu po zabiegu, a także większej satysfakcji pacjentów z opieki pielęgniarskieji pobytu w oddziale.

This thesis is dedicated to the role of the therapeutic communication in patients’ psychological preparation to the serious orthopedic surgeries. The aim of the study was an evaluation of the influence of a psychological preparation of patients after the knee replacement surgery and its influence on the post surgery period. Sixty patients took part in the study, they were all from the orthopedic and traumatic ward. Thirty of them were qualified to the experimental group and another thirty to the control group. The control group was prepared to the operation in the standard way while in the experimental group the nurse prepared an additional psychological and supporting conversation. In the experimental group a noticeable improvement of the post surgery period was observed in comparison to the control group. A difference in the speed of recovery was shown together with the usage of less amount of painkillers and sedatives. The self evaluation during the post surgery period was also improved together with better mood and greater satisfaction from the nurses’ care during the hospital treatment.
