The model care in eldery patients with right ventricular failure

Przewlekła niewydolność krążenia jest złożonym zespołem klinicznym związanym z wysoką chorobowością i śmiertelnością. To stan w którym serce jako pompa nie może zabezpieczyć wystarczającego dla potrzeb ustroju dopływu krwi do tkanek i narządów.Najczęstszymi przyczynami przewlekłej niewydolności serca w Polsce jest choroba niedokrwienna serca i nadciśnienie tętnicze. Zapadalność i chorobowość rośnie z wiekiem. Największa chorobowość dotyczy osób po 70 roku życia. Diagnostyka i leczenie pacjenta w starszym wieku jest trudniejsze, ponieważ obraz kliniczny wielu schorzeń ulega zmianie pod wpływem procesu starzenia; także zebranie wywiadu nastręcza trudności ze względu na zaburzenia słuchu, afazję czy zaburzenia pamięci. Celem terapii jest leczenie choroby będącej przyczyną przewlekłej niewydolności serca, profilaktyka i leczenie stanów odpowiedzialnych za zaostrzenie choroby. Bardzo ważne jest włączenie pacjenta i/lub opiekunów w proces leczenia. Przewlekła niewydolność krążenia stanowi istotny problem zdrowotny i społeczny, a koszty leczenia tej grupy pacjentów ciągle wzrastają. Celem mojej pracy jest przedstawienie problemów zdrowotnych jakie występują u pacjentów w starszym wieku z przewlekłą prawokomorową niewydolnością serca oraz wprowadzenie modelu opieki nad takim chorym, co pozwoli uzyskać oczekiwane efekty lecznicze i poprawić jakość życia pacjentów. W pracy opisano 8 problemów zdrowotnych. Określono cel opieki i postępowanie leczniczo-pielęgnacyjne.

Chronic heart failure is a complex clinical syndrome associated with a high morbidity and mortality. This is a condition in which the heart as a pump cannot secure enough for the system of blood supply to tissues and organs.The most common causes of chronic heart failure in Poland are coronary heart disease and hypertension. The incidence and the prevalence of heart failure increases with age. The highest morbidity relates to persons over 70 years of age. Diagnosis and treatment of older patients is more difficult because many clinical conditions altered by the aging process. In addition, an interview with the patient may be difficult because of the hearing impairment, aphasia or impaired memory. Chronic heart failure therapy is focused on treating the underlying disease as well the prevention and treatment of conditions responsible for exacerbation of the disease. Important is to incorporate the patient and/or their caregivers in the treatment process.Chronic heart failure is a major health and social problem, and the cost of treating this group of patients is still increasing.The aim of my work is to present health problems that occur in elderly patients with right ventricular failure and to propose such a model patients care, which will get the expected therapeutic effects and improve their quality of life. This paper describes eight health problems. Defines the objective of care and medical and nursing procedures.
