Physiotherapeutic treatment in a patient in a with Sinding Larsen Johansson's disease - a case study

Wstęp: Sinding-Larsen-Johannson jest rzadką chorobą stawu kolanowego. Ból w tym zespole występuje przy przebiegu więzadła rzepki przez jej wierzchołek. Chorobę można diagnozować za pomocą badań obrazowych jednak sama jej etiologia nie jest do końca znana.Cele pracy: Celem pracy była analiza i przedstawienie przypadku trzynastoletniej dziewczynki z chorobą Sindinga-Larsena-Johannsona oraz jej sprawności fizycznej po zastosowanej rehabilitacji.Materiały i metody: W pracy został opisany przypadek trzynastoletniej siatkarki, która odczuwała silny ból przy dystalnym końcu rzepki w obu kolanach. Pacjentkę oceniono na podstawie autorskiego kwestionariusza ankiety, badania ortopedycznego i badania USG.. Wykorzystano test FMS oraz testy funkcjonalne np. test Lachmana,Mc Murraya. Sprawdzono zakres ruchu w stawie kolanowym. Zbadano również siłę mięśniową kończyn dolnych oraz ich długość i obwody. Badanie zostało przeprowadzone po zakończonej rehabilitacji.Wyniki: W teście FMS pacjentka miała bardzo dobre wyniki, zakres ruchu w kolanach jest w pełni zachowany. Siła mięśniowa testowanych mięśni oceniona została na 5 według Lovetta. Mimo dobrych wynikóww przeprowadzanym badaniu pacjentka nie jest zadowolona z wyników rehabilitacji, ponieważ odczuwa nadal dolegliwości bólowe w obydwu kolanach.Wniosek: W zespole Sindinga-Larsena-Johannsona istotnym elementem terapii jest rehabilitacja oraz powstrzymanie się od aktywności sportowej. Nie zawsze jednak przynosi to satysfakcjonujące efekty.

Introduction: Sinding-Larsen-Johannson is a rare disease of knee joint. The pain in this syndrome occurs in transition of patella ligament through it’s apex of patella. The syndrome can be diagnosed through imaging studies, although it’s a etiology is not well known.Aim of the work: The work purposes were: analysis and introducing the case of thirteen year old girl with Sinding-Larsen-Johannson disease and her health state after the rehabilitation.Material and methods: In the thesis, the case of a thirteen year old volleyball player who felt severe pain at the distal end of the patella in both knees. The patient was described, based on proprietary inquiry form, orthopedic examination and USG examination. An FMS test was used, along with black-box testing e.g. Lachman test, Mc Murray test. The range of motion in the knee joint was checked. Muscle strength of the lower limbs as well as their length and circumferences were also examined. Examination was conducted after the finished rehabilitationResults: FMS showed that patient had very good results, range of motion in knees is fully preserved. Muscle power of tested muscles was rated 5, according to Lovett. Despite the god results, in conducted examination patient was not satisfied with the outcome of rehabilitation, as she still experiences pain.Conclusion: In the Sinding-Larsen-Johansson syndrome an important element of therapy is rehabilitation and abstaining from sports activity. However, it does not always bring satisfactory results.
