Asthma attack - rescue operations - case study

Wstęp. Stan astmatyczny może rozwinąć się u każdego chorego na astmę oskrzelową i stanowi stan zagrażający życiu chorego. Duża aktualność tematyki skłoniła autorkę do podjęcia badań własnych.Cel pracy. Celem pracy było przedstawienie obrazu klinicznego i działań ratunkowych prowadzonych przez ratowników medycznych na miejscu zdarzenia oraz w szpitalnym oddziale ratunkowych u chorej z napadem stanu astmatycznego.Opis przypadku. Przedstawiono przypadek napadu stanu astmatycznego u 81.letniej chorej. Chora została objęta działaniami diagnostyczno-terapeutycznymi prowadzonymi przez ratowników medycznych zarówno na miejscu zdarzenia (w domu chorej) jak i w szpitalnym oddziale ratunkowym. Właściwe i skuteczne działania ratowniczo-terapeutyczne pozwoliły na likwidację stanu astmatycznego w ciągu 3 godz. i uzyskanie przez chorą pełnej wydolności oddechowej. Wnioski. Do wystąpienia stanu astmatycznego może prowadzić szereg czynników, w tym infekcja dróg oddechowych, która właśnie występowała u opisywanej chorej. Szczególne znaczenie w diagnostyce ma gazometria krwi tętniczej wykonywana podczas trwania stanu astmatycznego, pozwala ona na rozpoznanie niewydolności oddechowej, ocenę jej nasilenia, a także monitorowania skuteczności prowadzonej terapii.

Introduction. The asthmatic attack may develop in any patient with bronchial asthma and is a life-threatening condition. High relevance of the subject led the author to undertake her own research. Objective of the work. The aim of the study was to present the emergency procedures carried out by paramedics on the scene and in the hospital emergency department on a patient with asthmatic attack.A case report. A case of asthmatic attack in a 81-year-old patient was presented. Diagnostic and therapeutic activities were carried out by paramedics both at the place of the accident (in the patient's home) and in the hospital emergency department. Proper and effective rescue and therapeutic actions allowed eliminating status asthmaticus within 3 hours and restoring full respiratory capacity by the patient.Conclusions. A number of factors may lead to the occurrence of asthmatic condition, including respiratory tract infection, which was the described case. Particular importance in the diagnostics is the arterial blood gasometry performed during the asthmatic stage, it allows the diagnosis of respiratory failure, assessment of its severity, and monitoring the effectiveness of the therapy.
