Perinatal period midwifery care of a newborn and a mother with gestational diabetes class g2- case study

Wstęp: Cukrzyca ciężarnych to często występujące powikłanie w ciąży. Istotne znacznie odgrywa wczesna diagnostyka oraz odpowiednie leczenie zapewniające wyrównanie glikemii, a tym samym zapobieganie wystąpienia powikłań u matki i dziecka. W terapii szczególne znaczenie ma edukacja pacjentki dotycząca samoopieki oraz uświadomienie o konsekwencjach nie przestrzegania zaleceń. W opiece okołoporodowej ważną rolę odgrywa położna, która przekazuje informacje, koryguje błędy oraz obserwuje i wspiera pacjentkę. Cel: Celem pracy było wyłonienie problemów zdrowotnych i pielęgnacyjnych ciężarnej z cukrzycą ciążową typu G2 oraz ustalenie celów i interwencji okołoporodowych wobec matki i jej dziecka.Materiał i metody: Podmiotem objętym badaniem była ciężarna lat 31 CII PII w 37 tygodniu powikłanej cukrzycą ciążową typu G2.W pracy zastosowano metodę indywidualnego przypadku oraz technikę wywiadu i analizy dokumentacji.Wyniki: W wyniku przeprowadzonego wywiadu i analizy dokumentacji medycznej określono problemy pielęgnacyjno-edukacyjne oraz sprecyzowano plan opieki nad pacjentką. Wnioski: Położna sprawująca opiekę powinna wykazać się znajomością zagadnień dotyczących cukrzycy ciążowej. Podejmowane przez nią działania muszą obejmować ocenę stanu wiedzy pacjentki oraz korektę ewentualnych błędów. Zadaniem położnej jest także monitorowanie stanu biopsychospołecznego pacjentki, udzielanie jej wsparcie oraz obserwacja dobrostanu dziecka.

Introduction: Gestational diabetes is a complication often occurring during pregnancy. The most important are early diagnosis and appropriate treating, that effects in a stable level of glycaemia, eliminating the risk of further mother and child’s health complication. An essential part of the therapy is education of the patient on self-care and helping her realize what are the consequences of neglecting the given prescriptions. The midwife plays a crucial role in the perinatal period. She transfers information, corrects mistakes, observes and supports the patient.Objective: The aim of this work was to bring to light the health problems and nursing problems of a pregnant woman with gestational diabetes class G2. And furthermore, to set the goals and interventions that need to be undertaken in the perinatal period towards the mother and child. Material and methods: The subject of the study was a 31 year old pregnant woman GII PII in the 37th week with gestational diabetes class G2. The method of individual case study and the techniques of interview and document analysis were used in this study. Results: As a result of medical interview and document analysis, educational and nursing problems were put forward, and a plan of patient care was established. Conclusions: A midwife providing care for her patient should possess sufficient knowledge about gestational diabetes. The interventions undertaken by her have to include an evaluation of the state of knowledge presented by the patient and a correction of possible mistakes. Her responsibilities also include monitoring of the patient’s biopsychosocial condition, offering her support and observing the child’s well-being.
