Organization of Information in the Polish Healthcare System from the Users’ Perspective

Praca analizuje funkcjonowanie opieki zintegrowanej w polskim systemie ochrony zdrowia, ze szczególnym uwzględnieniem roli procesów cyfryzacji. Przedstawiono genezę i rozwój systemu, jego współczesną strukturę oraz wyzwania związane z demografią i organizacją świadczeń. W części teoretycznej omówiono kształtowanie systemu opieki zdrowotnej w Polsce - od rozwiązań w II Rzeczypospolitej, poprzez centralistyczny model okresu PRL, aż po reformy w III RP i wprowadzenie modelu ubezpieczeniowego. Pokazano, że historyczne zmiany miały często fragmentaryczny charakter, co prowadziło do braku spójności i nieskutecznej koordynacji opieki. Na tym tle zintegrowana opieka zdrowotna jawi się jako sposób na uporządkowanie i uspójnienie systemu. Szczególną uwagę poświęcono wdrażaniu Elektronicznej Dokumentacji Medycznej, e-recept, e-skierowań i Internetowego Konta Pacjenta jako narzędzi poprawy koordynacji opieki. W pracy podjęto także temat ochrony danych osobowych w świetle RODO i perspektywy Europejskiej Przestrzeni Danych Zdrowotnych. Część empiryczna, oparta na badaniu ankietowym, koncentruje się na analizie kompetencji cyfrowych osób starszych - kluczowej grupy odbiorców usług zdrowotnych - oraz na barierach ich udziału w systemie e-zdrowia. Wyniki wskazują na konieczność dalszych działań edukacyjnych i rozwojowych, aby zwiększyć dostępność zintegrowanej, cyfrowej opieki w Polsce.

The thesis analyzes the functioning of integrated care in the Polish healthcare system, with particular emphasis on the role of digitalization processes. It presents the origins and development of the system, its current structure, and challenges related to demographics and the organization of services. The theoretical part discusses the evolution of the healthcare system in Poland - from the solutions of the Second Polish Republic, through the centralized model of the socialist period, to the reforms of the Third Republic and the introduction of the insurance-based model. It is shown that historical changes were often fragmentary, leading to a lack of coherence and ineffective coordination of care. Against this background, integrated healthcare appears as a way to organize and streamline the system. Special attention is devoted to the implementation of Electronic Medical Records, e-prescriptions, e-referrals, and the Internet Patient Account as tools for improving care coordination. The thesis also addresses issues of personal data protection in the context of the GDPR and the prospects of the European Health Data Space. The empirical part, based on a questionnaire survey, focuses on the analysis of the digital competences of older adults - a key group of healthcare service recipients - and the barriers to their participation in the e-health system. The findings indicate the need for further educational and developmental efforts to increase the accessibility of integrated, digital healthcare in Poland.
