Model of patient care with burn disease

Wstęp: Choroba oparzeniowa jest stanem zagrażającym życiu i wymaga podjęcia natychmiastowych działań. Na jakość leczenia pacjenta we wstrząsie oparzeniowym wpływ ma szybkie i skoordynowane działanie personelu medycznego oraz jego współpraca i doświadczenie w tej dziedzinie. Poza resuscytacją płynową istotnym elementem terapii jest leczenie ran oparzeniowych oraz terapia żywieniowa Cel: Celem pracy było opracowanie modelu opieki nad pacjentem z chorobą oparzeniową, w oparciu o teorię Dorothy Orem z uwzględnieniem diagnoz pielęgniarskich, celu opieki, interwencji oraz oczekiwanych wyników opieki.Materiały i metody: W pracy posłużono się metodą analizy piśmiennictwa naukowego oraz obserwacji w praktyce pielęgniarskiej.Wyniki i wnioski: Kluczowe diagnozy pielęgniarskie w opiece nad pacjentem w chorobie oparzeniowej to ryzyko rozwoju wstrząsu hipowolemiczego, niewydolności wielonarządowej, ból ran oparzeniowych, zaburzenia metaboliczne i gospodarki wodno-elektrolitowej, możliwość zakażenia ran, powikłania wynikające z unieruchomienia a także niepokój pacjenta w obawie przed oszpeceniem. Podjęcie działań pielęgnacyjno - terapeutycznych przedstawionych w opracowanym modelu opieki ma za zadanie zmniejszenie ryzyka wystąpienia powikłań podczas leczenia oraz poprawę jakości życia pacjenta z chorobą oparzeniową.

Introduction: Burn disease is a life-threatening condition and, as such, requires taking immediate action. The quality of treatment of a patient suffering from burn shock is influenced by rapid and coordinated action by medical staff and its cooperation and expertise in this field. In addition to fluid resuscitation, the treatment of burn wounds and nutritional therapy are also important elements of the therapy. Aim: The aim of this study was to develop a model of care for a patient with burn disease, based on the theory of Dorothy Orem, taking into account nursing diagnoses, the aim of the care, interventions and expected outcomes of care.Materials and Methods: The method used in this study was the analysis of scientific literature as well as observation in nursing practice.Results and conclusions: Key nursing diagnoses in providing care to a patient suffering from burn disease are: risk of hypovolemic shock as well as multiple organ failure, pain in burn injuries, metabolic and water-electrolyte disorders, possible wound infections, complications resulting from immobilization and the patient's anxiety and fear of disfigurement. Implementation of nursing and therapeutic activities, presented in the developed model of care, allows to reduce the risk of complications during treatment and improve quality of life of a patient with burn disease.
