Nursing care of a patient with Paget's cancer - case study

WSTĘP. Rak Pageta to rak śródnaskórkowy, występujący w okolicach brodawki sutkowej, któremu towarzyszyć może wewnątrzprzewodowy lub przewodowy naciekający rak piersi. Dotyka w głównej mierze kobiet po menopauzie. Pierwszym objawem jest powstanie w okolicy brodawki zmian o typie owrzodzenia, strupa, nadżerki zaczerwienienia. Wybór odpowiedniej metody leczenia zależy od stopnia zaawansowania choroby. Jedną z nich jest radioterapia. CEL PRACY. Sformułowanie diagnoz pielęgniarskich w oparciu o rozpoznane problemy zdrowotne i pielęgnacyjne pacjentki z rakiem Pageta, a także opracowanie indywidualnego planu opieki dostosowanego do jej potrzeb.MATERIAŁ i METODY. W pracy wykorzystano metodę indywidualnego przypadku 59-letniej pacjentki z chorobą Pageta brodawki sutkowej w trakcie radioterapii. Zastosowane techniki to: obserwacja, wywiad, pomiary podstawowych parametrów oraz analiza dokumentacji medycznej. W celu oceny stopnia akceptacji choroby przez pacjentkę użyto skali AIS.WYNIKI. Pierwsze objawy choroby pojawiły się u pacjentki w 2019 roku, diagnozę potwierdzono w 2020. Początkowo u pacjentki zastosowano leczenie miejscowe, następnie z powodu nieskuteczności – chirurgiczne z uzupełniającą radioterapią. Ocena stanu bio – psycho – społecznego pacjentki, pozwoliła na rozpoznanie najważniejszych problemów zdrowotnych i pielęgnacyjnych. Za najistotniejsze uznano: występowanie odczynu popromiennego, braku apetytu, zaparcia. Mimo, że stopień akceptacji choroby był u pacjentki wysoki, odczuwała niepokój z powodu złego stanu zdrowia oraz prowadzonego leczenia.WNIOSKI. W efekcie oceny stanu pacjentki postawiono 10 diagnoz pielęgniarskich. Za najistotniejsze uznano te będące skutkami ubocznymi stosowanej radioterapii. Wdrożenie zaplanowanych interwencji, pozwoliło na złagodzenie dolegliwości oraz poprawę samopoczucia pacjentki.

INTRODUCTORY REMARKS. Paget's cancer is an intra-epidermal carcinoma that occurs around the nipple that may be accompanied by intra-ductal or ductal infiltrating breast cancer. It mainly affects postmenopausal women. It is initial symptom is the formation of ulcerative lesions, scabs, erosions, and redness in the nipple area. The choice of the appropriate treatment method depends in the stage of the disease. One of them is radiotherapy.AIM OF THE DISSERTATION. Formulate nursing-oriented diagnoses basing on the identified health and care-related problems of a female patient suffering from Paget's cancer, as well as to develop an individual treatment plan adjusted to her needs. MATERIAL AND METHODS. The study utilizes the method of an individual case study of a 59-year-old female patient suffering from Paget's disease of the nipple during radiotherapy. The techniques used are: observation, interview, measurements of basic parameters, as well as the analysis of the medical records. In order to assess the degree of acceptance of the disease by the patient, the AIS scale has been used. RESULTS. The patient first showed symptoms of disease in 2019, the diagnosis was confirmed in 2020. The patient was initially treated with local therapy, then due to ineffectiveness, surgical treatment with complementary radiotherapy. Assessment of the patient’s bio – psycho – social condition, allowed to identify the most important health and nursing problems. The most important have been: radiation reaction, lack of appetite, and constipation. Although the patient’s acceptance of her illness was high, she felt anxious about her state of health and the treatment provided. CONCLUSIONS. As a result of the assessment of the patient’s condition, 10 nursing diagnoses have been made. . The most important have been those being connected with side effects of radiotherapy. The implementation of planned interventions allowed to alleviate the symptoms and improve the patient's well-being.
