Care of the mechanically ventilated patient, infected by Clostridium difficile and by methicillin-resistant Staphylococcus aureus.

Specyfika oddziału intensywnej terapii wiąże się ze szczególnym profilem osób hospitalizowanych oraz stosowaniem inwazyjnych technik diagnostyczno-terapeutycznych. W toku długotrwałej hospitalizacji dochodzi często do powikłań infekcyjnych, które powodują zagrożenie zdrowia lub życia. W pracy omówiono specyfikę oddziałów intensywnej terapii oraz problem zakażeń szpitalnych w Oddziale Intensywnej Terapii. Przedstawiono charakterystykę wybranych zakażeń: infekcji Clostridium difficile oraz metycylinoopornym Staphylococcus aureus, z uwzględnieniem środków zapobiegania zakażeniom tymi patogenami. Zaprezentowano opis przypadku 50-letniego mężczyzny z mikroskopowym zapaleniem naczyń i krwawieniem do pęcherzyków płucnych oraz niewydolnością nerek, u którego w trakcie hospitalizacji dochodzi do zakażenia patogenami Clostridium difficile oraz metycylinoopornym Staphylococcus aureus. Przedstawiono przebieg hospitalizacji oraz ocenę stanu pacjenta. Zaprezentowano proces pielęgnowania pacjenta, ze szczególnym uwzględnieniem problemów pielęgnacyjnych występujących u chorych zainfekowanych Clostridium difficile oraz metycylinoopornym Staphylococcus aureus. W procesie pielęgnowania uwzględniono także problem ryzyka przeniesienia zakażenia na innych pacjentów.

Intensive care units are characterized by the special profiles of patients hospitalized there as well as by the usage of invasive diagnostic and therapeutic techniques. The process of long term treatment often results in many infection complications which may be the threat for the patients’ health or even their life. In this thesis the specifics of the intensive care units and the problem of nosocomial infections which may occur there have been discussed. The characteristics of a two infections have been presented. These are: the infections caused by Clostridium difficile and by Staphylococcus aureus which is methicillin-resistant. Also the ways to prevent the infections caused by these pathogens have been discussed. It has been also described the case study of 50 year old man who suffered from the microscopic vasculitis, bleeding alveolar and the renal failure. During the treatment on intensive care unit, the patient was also diagnosed with Clostridium difficile and methicillin-resistant Staphylococcus aureus infections. The presentation of the whole treatment process, the hospitalization and the assessment of the patient’s condition have been done. The special attention has been paid to the nursing problems which are characteristic for this kind of infections.
