Assessment of functional capacity and dependence on care in elderly people receiving outpatient clinical care

Wstęp: Wraz z wiekiem osób starszych i postępującymi zmianami w organizmie dochodzi do zmniejszenia sprawności i wydolności samoopiekuńczych, które stają się wyznacznikiem zapotrzebowania na pomoc ze strony innych osób. Właściwa ocena sprawności funkcjonalnej i zależności od opieki jest punktem wyjścia do zidentyfikowania obszarów codziennego funkcjonowania, w których występuje największy deficyt.Cel pracy: Celem pracy była ocena sprawności funkcjonalnej i zakresu zależności od opieki osób starszych leczonych ambulatoryjnie. Materiał i metody: Badaniem objęto 151 osób starszych leczonych w Niepublicznym Zakładzie Opieki Zdrowotnej w województwie małopolskim. W badaniach wykorzystano metodę sondażu diagnostycznego, stosując następujące narzędzia badawcze: kwestionariusz ankiety własnego autorstwa, dotyczący charakterystyki demograficzno-społecznej oraz narzędzia standaryzowane: AMTS, GSOD, SHARE-FI, CDS, MNA, I-ADL i P-ADL.Wyniki: Ocena badanej grupy osób starszych przy pomocy skali I-ADL oraz P-ADL wykazała dobry poziom sprawności funkcjonalnej. Jedynie 13,9% osób starszych wg skali I-ADL oraz 15,2% wg skali P-ADL osiągnęło wynik świadczący o zaburzeniach funkcjonalnych. U zdecydowanej większości badanych (84,8%) stwierdzono niski poziom zależności od opieki.Wnioski: Przeprowadzone badanie wykazało istotną zależność między stopniem sprawności funkcjonalnej i poziomem zależności od opieki, a ryzykiem wystąpienia zespołu kruchości, depresji, niedożywienia oraz liczbą posiadanych chorób i ilością stosowanych leków. Spośród ocenianych czynników demograficzno-społecznych wiek, poziom wykształcenia i stan cywilny miał istotny wpływ na stopień sprawności funkcjonalnej i poziom zależności od opieki. Struktura zamieszkania i sytuacja materialna nie miała istotnego wpływu na stopień sprawności funkcjonalnej i poziom zależności od opieki.

Introduction: Along with the process of ageing and the progressing changes in the organism, elderly people become affected by reduced fitness and self-care capacity, which implies the necessity of seeking assistance from other people. A proper evaluation of functional capacity and dependence on care is the starting point for identifying the most significant deficits in activities of daily living.Aim of the study: The aim of the study was to assess functional capacity and the scope of dependence on care in elderly people receiving outpatient clinical care.Material and methods: The study group consisted of 151 elderly people treated at a private health care centre in the Małopolskie Province. The study used the method of diagnostic survey with the following research tools: an own survey questionnaire concerning the demographic and social characteristics and some standardised tools, i.e. AMTS, GSOD, SHARE-FI, CDS, MNA, I-ADL and P-ADL.Results: Assessment of the study group of elderly people using the I-ADL and P-ADL scales showed a good level of functional capacity. Only 13.9% of elderly people, acc. to the I-ADL scale, and 15.2% acc. to the P-ADL scale had a score indicating functional disorders. In a vast majority of the study subjects (84.8%) very low level of dependence on care was found.Conclusions: The study revealed a significant correlation between, on the one hand, the degree of functional capacity and the level of dependence on care and, on the other hand, the risk of frailty syndrome, depression, malnourishment and the number of diseases and the number of medicines taken. Among the demographic and social factors that were assessed, the level of education and the marital status had a significant effect on the level of functional capacity and the level of dependence on care. The place of habitation and the material situation did not have a significant effect on the level of functional capacity and the level of dependence on care.
