Patient with obesity, arterial hypertension and type 2 diabetes - case study

Wstęp: Otyłość jest bardzo często spotykanym problemem zdrowotnym w naszym społeczeństwie, jednak wiele osób nie zdaje sobie sprawy z faktu, iż jest to choroba i powinna być leczona oraz jak wiele negatywnych konsekwencji niesie za sobą ten stan. Praca ta przedstawia charakterystykę tej choroby ze szczególnym uwzględnieniem powikłań, jakimi są nadciśnienie tętnicze oraz cukrzyca typu 2.Cel pracy: Celem pracy było zaplanowanie i zrealizowanie opieki nad pacjentką otyłą z nadciśnieniem tętniczym oraz cukrzycą typu 2 na podstawie studium indywidualnego przypadku. Materiał i metody: Podczas sprawowania opieki nad pacjentką wykorzystano metodę indywidualnego przypadku. Zastosowano technikę wywiadu, rozmowy, pomiaru, obserwacji oraz analizy treści dokumentacji medycznej. Do powstania pracy wykorzystano także narzędzia takie, jak: arkusze do gromadzenia danych o pacjencie, kwestionariusz oceny jakości życia SF – 36 oraz skala oceny bólu.Wyniki: Zastosowane techniki pozwoliły na ocenę stanu biopsychospołecznego pacjentki podczas 4. wizyt środowiskowych oraz postawienie 10 diagnoz pielęgniarskich i określenie celu opieki, a także zaplanowanie interwencji do każdej z nich. Wnioski: Choroba oddziałuje negatywnie na wiele aspektów życia pacjentki. Chora ma problem z przestrzeganiem zaleceń zmiany nawyków żywieniowych, co nie daje zadawalających efektów w utracie masy ciała. Jednak poznanie dokładniej istoty chorób i ich powikłań zmotywowało pacjentkę do większego dbania o zdrowie. Pacjentka zostaje pod kontrolą lekarzy specjalistów.

Introduction: Obesity is very common health problem in our society, but many people do not realize that it is a disease and should be treated and how many negative consequences carries this state. This study presents the characteristics of this disease with particular emphasis on complications, which are arterial hypertension and type 2 diabetes.Aim of the study: The aim of this study was to plan and realize care of the patient with obesity, arterial hypertension and type 2 diabetes based on case study.Material and methods: The method of the individual case study was used during patient care. There were used the techniques of: interview, talk, measurement, observation and analysis of medical records. To create this study were also used tools such like: the health condition questionnaire and quality of life questionnaire and also the pain assessment scales. Results: The techniques allowed the biopsychosocial assessment of the patient during 4 environmental visits and place 10 nursing diagnoses and determine the care and planning interventions for each of them.Conclusions: The disease has a negative effect on many aspects of life of the patient. Patient has a problem with adaptation to change eating habits, which does not give rewarding results in weight loss. However, understanding precisely the point of diseases and their complications motivated the patient to a better self-care for the health. Patient is under the care of specialists.
