Administrative change of first or last name

Tematem niniejszej pracy jest przedstawienie administracyjnej procedury zmiany imienia i nazwiska. Praca składa się z pięciu rozdziałów. Pierwszy wprowadzający rozdział prezentuje genezę imienia i nazwiska oraz historyczny zarys reguł zmiany imienia i nazwiska. Drugi rozdział natomiast poświęcony jest zagadnieniu przesłanek zmiany imienia i nazwiska. W trzecim rozdziale zaprezentowana została procedura administracyjna. Czwarty rozdział skupia się na weryfikacji decyzji w sprawie zmiany imienia i nazwiska. Ostatni rozdział prezentuje zasady zmiany imienia i nazwiska w wybranych krajach Unii Europejskiej.

The subject of this thesis is to describe a change of first or last name in administrative procedure. The thesis consists of five chapters. The first introductory chapter presents the genesis of first and last name and historical outline of rules for change of first or last name. The second chapter, however, deals with the issue of name change conditions. In the third chapter administrative procedure is presented. The forth chapter focuses on a review of an name change decision. The last chapter presents rules for change of first or last name in selected countries of the European Union.
