Diagnostic imaging of gastric cancer - case study

W 2017 roku rak żołądka wywodzący się z komórek nabłonkowych błony śluzowej żołądka znalazł się u mężczyzn na 4. miejscu pod względem śmiertelności na nowotwory złośliwe. Odsetek zachorowań na rak żołądka wynosi 4% u mężczyzn oraz 2,4% u kobiet, mimo tego szansa pacjenta na 5-letnie przeżycie wynosi zaledwie 20%.W pierwszej części pracy zawarto ogólne zagadnienia związane z anatomią i fizjologią żołądka oraz podstawowe informacje dotyczące raka żołądka – klasyfikacja, obraz kliniczny, ocena stopnia zaawansowania, diagnostyka obrazowa obejmująca badanie endoskopowe, badanie radiologiczne z podaniem środka cieniującego, tomografię komputerową i rezonans magnetyczny. Przedstawiono także metody leczenia w przypadku zdiagnozowania raka żołądka.W kolejnej części przedstawione zostało postepowanie diagnostyczne na przykładzie 35-letniego pacjenta z nowotworem złośliwym żołądka zawierające badanie endoskopowe górnego odcinka przewodu pokarmowego oraz tomografię komputerową żołądka.W podsumowaniu opisano schematycznie najważniejsze etapy postępowania diagnostycznego w przypadku raka żołądka prowadzące do rozpoznania oraz określenia stopnia zaawansowania nowotworu złośliwego.

In 2017, gastric cancer originating from the epithelial cells of the gastric mucosa was ranked 4th in terms of mortality from malignant neoplasms. The incidence of gastric cancer is 4% in men and 2.4% in women, yet the chance of a patient surviving 5 years is only 20%.The first part of the work covers general issues related to the anatomy and physiology of the stomach as well as basic information on gastric cancer - classification, clinical picture, stage assessment, diagnostic imaging including endoscopic examination, radiological examination with contrast medium, computed tomography and magnetic resonance imaging. Treatment methods for the diagnosis of gastric cancer are also presented.The next section presents a diagnostic procedure based on the example of a 35-year-old patient with gastric cancer, including endoscopic examination of the upper gastrointestinal tract and computed tomography of the stomach.The summary describes schematically the most important stages of the diagnostic procedure in the case of gastric cancer leading to the diagnosis and determination of the stage of the malignant neoplasm.
