Emergency Management with a Pediatric Patient in Hypothermia - Case Study

Wstęp: Człowiek jest organizmem stałocieplnym. Aby organizm funkcjonował w sposób prawidłowy temperatura narządów wewnętrznych musi utrzymywać się na stałym poziomie – ok. 37 ºC. Istnieją cztery mechanizmy utraty ciepła: kondukcja, konwekcja, promieniowanie i parowanie. W zależności od stopnia hipotermii pacjent prezentuje różne objawy, dlatego bardzo ważna jest jego kompleksowa ocena zgodnie ze zmodyfikowanym schematem ABCDE. W tym przypadku nie należy zapominać o zbadaniu temperatury. Na rynku są dostępne różne metody oraz narzędzia przeznaczone do pomiaru tego parametru, jak również różne metody leczenia skrajnego wychłodzenia organizmu, w tym oksygenacja pozaustrojowa typu ECMO.Cel pracy: Celem pracy jest przedstawienie jak efektywna współpraca wszystkich podmiotów leczniczych oraz ludzi udzielających pierwszej pomocy na miejscu zdarzenia wpływa na przeżywalność pacjentów w hipotermii głębokiej.Opis przypadku: W pracy został ukazany przypadek 27-miesięcznego chłopca, który przeżył epizod asystolii w przebiegu hipotermii głębokiej.Wyniki: Chłopcu na miejscu zdarzenia została udzielona pierwsza pomoc w algorytmie BLS, następnie pacjent został zabezpieczony przez ratowników medycznych. Podjęto decyzję o przetransportowaniu go do szpitala z możliwością leczenia ECMO. Podsumowanie i wnioski: Hipotermia ma wiele postaci. Przypadek opisywanego dziecka należy do rzadkości, lecz ukazuje, że granica nieodwracalnej hipotermii nie została jeszcze zbadana, dlatego należy postępować zgodnie z maksymą „nikt nie jest martwy, dopóki nie jest ciepły i martwy”.
Introduction: Humans are homeotherms. To maintain proper functioning, a human body needs to maintain its temperature at the right level, i.e. circa 37 ºC. There are four mechanisms of heat loss: conduction, convection, radiation and evaporation. Depending on the level of hypothermia a patient presents different symptoms. That is why it is crucial to examine the patient thoroughly based on the modified ABCDE assessment. One must not forget to measure the patient’s temperature. On the market there are various methods and tools designed for measuring temperature, as well as for treating severe hypothermia, including extracorporeal membrane oxygenation (ECMO). The aim of the work: The aim of the work is to present how effective co-operation of all medical units and people delivering first aid at the place of the event influences the chances of survival of a patient with deep hypothermia. Case study: The work presents the case of a 27-month-old boy, who survived an asystole episode in the course of deep hypothermia.Results: The boy received first aid at the place of the event in BLS algorithm. Next the patient was secured by paramedics. A decision was made to transport the boy to a hospital, where ECMO was used.Summary and conclusions: Hypothermia comes in many forms. The case of the described child is among the rarest, but it shows that the threshold of irreversible hypothermia still has not been fully examined. That is why the recommended approach is: „no one is dead until warm and dead.”
