Ocena systemu pomocy dla kobiet z inkontynencją

Cel: Celem pracy była analiza systemu pomocy kobietom z problemem nietrzymania moczu (NM), a także próbowano wskazać wady i niedoskonałości oraz ich przyczyny w obecnym jego funkcjonowaniu i potencjalne kierunki zmian na etapie dotarcia do osób z grup ryzyka i wczesnego diagnozowania.Wyniki: Wykazano, że 70,2% badanych nie posiadało wiedzy o problemie NM przed pojawieniem się objawów tej dolegliwości. Pacjentki z wysiłkowym nietrzymaniem moczu (WNM) zgłaszały się do lekarza średnio po 4 latach, z naglącym nietrzymaniem moczu (NNM) po 3 latach, z mieszaną postacią nietrzymania moczu (MNM) po 6 latach, a z nietrzymaniem moczu z przepełnienia po 10 latach od wystąpienia pierwszych objawów. Istotny okazał się fakt, że kobiety, których motywacją do leczenia było nasilenie objawów zgłaszały się do lekarza po więcej niż 3 latach. Ponad połowa (56,7%) badanych zarówno z I, jak II grupy uważała, że późno zgłosiła się do lekarza z powodu przeświadczenia, iż nietrzymanie moczu jest zjawiskiem normalnym, związanym z wiekiem, którego nie leczy się. Dodatkowo 11,5% z nich twierdziło, że w takim przekonaniu utwierdzał je lekarz. Większość ankietowanych (89,4%) łagodziła objawy nietrzymania moczu bez pomocy lekarza. Istotne statystycznie czynniki ryzyka po których wystąpiło NM to: poród (25,5%), operacja w obrębie podbrzusza (19,14%), menopauza (44%), oraz obciążająca praca fizyczna (9,2%). Wykazano istotny związek między rodzajem NM potwierdzonym badaniem urodynamicznym, a deklaracjami pacjentek (kwestionariusz Gaudenza).Wnioski: Należy dotrzeć do grup ryzyka w celu uwrażliwienia tych osób na możliwość zaistnienia objawów nietrzymania moczu w przyszłości, co pozwoliłoby na wdrożenie wczesnej profilaktyki NM oraz skłoniłoby te osoby do podjęcia leczenia.
Objective: This paper analyzed the help system for women with Urinary Incontinence (UI), and attempts to point out shortcomings and imperfections and their causes in it’s current functioning and potential directions for change in the reach out and early diagnosis stages.Material and methods: The study was performed using the method of diagnostic enquiry by means of questionnaire and interview among two groups of patients. The first group of 80 women aged 47-79 years, consisted of members of the Association of People with urinary incontinence "Uroconti". The second group consisted of 61 female patients aged 26-81 years who reported to the Urodynamic Laboratory of Gynecology and Oncology Clinic at the University Hospital in Krakow.Results: 70.2% of respondents had no knowledge about the problem of UI before symptoms of this ailment appeared.Patients with stress urinary incontinence (SUI) reported to the doctor an average after 4 years, patients with urgency urinary incontinence (UUI), after 3 years, patients with a mixed form of urinary incontinence (MUI), after 6 years, and overflow incontinence, after 10 years from the appearance of first symptoms. The fact that women, for whom the intensification of symptoms was the motivation for treatment, reported to the doctor after more than 3 years, turned out to be significant. More than half (56.7%) of both I, and II groups believed that they have reported late to see the doctor, because of the belief that incontinence is a normal age-related phenomenon, and is not treated. Most respondents (89.4%), controlled symptoms of urinary incontinence without the help of a doctor. The study showed such statistically significant risk factors of UI as: childbirth (25.5%), surgery within the abdomen (19.14%), menopause (44%), and hard physical work (9.2%). A significant link between the type of UI confirmed by urodynamic testing, and the declarations of the patients (Gaudenz), has been demonstrated. Conclusion: Reach out actions targeting groups at risk should be undertaken in order to sensitize them on the possibility of emergence of urinary incontinence symptoms in the future, this would allow the implementation of early UI prevention, and would encourage these people to undertake treatment.
