Physiotherapeutic treatment of a patient with a rheumatoid hand - a case study.

Reumatoidalne zapalenie stawów (RZS) to przewlekła układowa choroba tkanki łącznej o podłożu autoimmunologicznym i postępującym charakterze. Zmiany zapalne z towarzyszącym obrzękiem oraz bólem, głównie lokalizują się w stawach rąk i stóp, prowadząc z czasem do powstania deformacji, a tym samym ograniczenia funkcji manualnych, inwalidztwa i obniżenia jakości życia. Rehabilitacja pacjenta obok farmakoterapii i psychoterapii powinna stanowić niezbędny element kompleksowej opieki nad chorym.Celem pracy było przedstawienie procesu diagnostyczno-terapeutycznego u pacjentów z objawami ręki reumatoidalnej oraz podkreślenie roli fizjoterapii w usprawnianiu stawów rąk, poprawie zakresu ruchomości, a tym samym zwiększenia sprawności funkcjonalnej oraz jakości życia chorych.Praca ma charakter kazuistyczny. W pierwszej części przedstawiono etiopatogenezę, dane epidemiologiczne, obraz kliniczny, kryteria rozpoznania, metody diagnostyki i leczenia choroby, a także najczęstsze metody fizjoterapeutyczne stosowane w przebiegu RZS. W drugiej części zamieszczono opis przypadku 60-letniej kobiety, z zaawansowaną postacią RZS, z objawami ręki reumatoidalnej. Na podstawie wyników badań oraz oceny stanu fizykalnego przeprowadzono kompleksową rehabilitację, uzyskując poprawę w zakresie ruchomości stawów rąk oraz polepszenie sprawności ogólnej pacjentki. Najkorzystniejszymi metodami fizjoterapeutycznymi w przypadku ręki reumatoidalnej są ćwiczenia sprawności manualnej w połączeniu z masażem klasycznym oraz termoterapią.

Rheumatoid arthritis (RA) is a chronic systemic connective tissue disease with autoimmune nature and progressive character. Inflammatory changes accompanied by swelling and pain, are mainly located in the joints of the hands and feet, leading to the formation of deformities over time which cause the limitation of manual functions, disability and it lowers the quality of life. Rehabilitation of the patient next to pharmacotherapy and psychotherapy should be an indispensable element of comprehensive care of the patient.The aim of the bachelor's thesis was to present the diagnostic and therapeutic process in patients with rheumatoid hand symptoms and to emphasize the role of physiotherapy in improving joints of the hands, improving the range of mobility, and thus increasing the functional efficiency and quality of patients life.The BA thesis is casuistic. The first part presents the etiopathogenesis, epidemiological data, clinical picture, criteria for diagnosis, methods of diagnosis and treatment of the disease, as well as the most common physiotherapeutic methods used in the course of RA. The second part contains a description of a 60-year-old woman afflicted with advanced form of RA and with symptoms of rheumatoid hand. Based on the results of the tests and the assessment of the physical condition, comprehensive rehabilitation was carried out. That resulted in improvements in the mobility of the joints of the hands and improvement in the overall patient efficiency. The most beneficial physiotherapeutic methods in the case of rheumatoid hand are manual exercises combined with classical massage and thermotherapy.
