Care of a patient with deep infiltrating endometriosis - a case study.

Wstęp: Endometrioza to choroba przewlekła dotykająca 10-15% kobiet w wieku rozrodczym, powodująca m.in stan zapalany i ból miednicy poprzez występowanie tkanki błony śluzowej macicy poza jej eutopową lokalizacją. Złotym standardem diagnostyki endometriozy jest laparoskopia z potwierdzeniem histopatologicznym. Leczenie polega na łagodzeniu objawów endometriozy poprzez farmakoterapię lub operację. Położne odgrywają ważną rolę w opiece nad pacjentką z endometriozą – stawiają diagnozy pielęgniarsko-położnicze oraz planują interwencje i je realizują.Cel pracy: Celem pracy była analiza i przedstawienie przypadku pacjentki z endometriozą głęboko naciekającą, zakwalifikowanej do zabiegu wycięcia ognisk gruczolistości śródmacicznej z zatoki Douglas’a, hospitalizowanej w klinice Medicus Clinic we Wrocławiu w dniach 09-12.11.2021 roku, a także rozpoznanie diagnoz pielęgniarsko-położniczych i zaplanowanie interwencji. Materiały i metody: W pracy wykorzystano metodę indywidualnego przypadku. Podmiotem badania była pacjentka ze zdiagnozowaną endometriozą głęboko naciekającą. Techniki użyte w pracy to: analiza dokumentacji medycznej pacjentki, wywiad z pacjentką, obserwacja stanu ogólnego, ginekologicznego i psychicznego pacjentki, wykonywanie pomiarów i badań.Wyniki: Pacjentka opisana w pracy zmaga się z endometriozą głęboko naciekającą. Doświadczyła objawów takich jak: ból związany z cyklem miesiączkowym, obniżone samopoczucie, dyspareunia. Na podstawie oceny stanu biopsychospołecznego u pacjentki zidentyfikowano 18 problemów pielęgniarsko-położniczych.Wnioski: Opieka nad kobietą z endometriozą głęboko naciekającą jest wyzwaniem dla zespołu terapeutycznego. Pomimo ogromu wiedzy o endometriozie nadal trwają badania nad jej istotą. Podczas hospitalizacji zapewniono pacjentce wsparcie emocjonalne, monitorowano parametry stanu ogólnego, realizowano zlecenia lekarskie oraz przeprowadzono edukację. Podjęte interwencje przyczyniły się do poprawy stanu zdrowia pacjentki oraz skutecznego rozwiązania problemów pielęgniarko-położniczych.

Introduction: Endometriosis is a chronic disease affecting 10-15% of women of reproductive age causing, among other things, inflammation and pelvic pain through the presence of endometrial tissue outside its eutopic location. The gold standard for diagnosing endometriosis is laparoscopy with histopathological confirmation. Treatment consists of relieving the symptoms of endometriosis through pharmacotherapy or surgery. Midwives have an important role in the care of patients with endometriosis - they make nursing and obstetric diagnoses and plan and implement interventions.Aim: The purpose of this study was to analyze and present the case of a patient with deeply infiltrating endometriosis qualified for surgery to remove the focus of intrauterine endometriosis from the sinus of Douglas hospitalized at Medicus Clinic in Wrocław on 09-12.11.2021, as well as to identify nursing and obstetric diagnoses and plan interventions.Materials and methods: This study used the individual case method. The subject of the study was a patient diagnosed with deep infiltrating endometriosis. The techniques used in the study were: analysis of the patient's medical records, interview with the patient, observation of the patient's general, gynecological and psychological condition, taking measurements and tests.Results: Described patient struggles with deep infiltrating endometriosis. She struggled with symptoms such as pain associated with her menstrual cycle, decreased mood, and dyspareunia. Based on an assessment of the patient's biopsychosocial status, 18 nursing and obstetric problems were identified. Conclusions: The care of a woman with deeply infiltrating endometriosis is a challenge for the treatment team. Despite the vast amount of knowledge about endometriosis, research into its nature is still ongoing. During hospitalization, emotional support was provided to the patient, parameters of her general condition were monitored, medical orders were carried out and education was provided. The interventions taken contributed to the improvement of the patient's condition and successfully solved the nursing and obstetric problems.
