Wstęp: Choroba Hashimoto jest najczęściej występującym typem pierwotnej niedoczynności tarczycy. Choroba ta polega na zaburzeniu odpowiedzi immunologicznej i obecności przeciwciał przeciw tyreoperoksydazie oraz tyreoglobulinie. Część teoretyczna pracy przedstawia opis kliniczny przewlekłego autoimmunologicznego zapalenia tarczycy. W części zawierającej indywidualny model opieki zawarto diagnozy oraz interwencje pielęgniarskie.Cel pracy: Stworzenie indywidualnego modelu opieki nad pacjentką z chorobą Hashimoto.Materiał i metody: Praca została przygotowana z wykorzystaniem metody indywidualnego przypadku.Wyniki: Główne problemy zdrowotne pacjentki dotyczyły sfery fizycznej. Powodzenie podjętych działań, w przebiegu procesu pielęgnowania, zależało od wielokierunkowego podejścia do występujących u pacjentki problemów zdrowotnych. Stan zdrowia pacjentki w momencie zakończenia opieki uległ poprawie.Wnioski: Wykorzystanie metody indywidualnego przypadku umożliwiło przygotowanie kompletnego, indywidualnego modelu opieki nad chorą z przewlekłym autoimmunologicznym zapaleniem tarczycy. Analiza piśmiennictwa umożliwiła przedstawienie informacji oraz pogłębienie wiedzy na temat choroby Hashimoto.

Introduction: Hashimoto's disease is the most common type of primary hypothyroidism. This disease is a disorder of the immune responses and the presence of antibodies against thyroid peroxidase and thyroglobulin. The theoretical part of the work presents the clinical description of chronic autoimmune thyroiditis. In the part containing an individual model of care, included nursing diagnoses and interventions.The aim of the study was to create an individual model of care a patient with Hashimoto's disease.Materials and methods: The work was prepared using the method of the individual case.Results: The mainly health problems associated with the patient's physical realm. The success of the actions, which has been taken in the course of the nursing process depended on the multidirectional approach to occurring health problems. The health status of the patient has been improved at the end of care.Conclusions: The use of the method of the individual case allowed the preparation of the complete, individual model of care for a patient with chronic autoimmune thyroiditis. The analysis of literature allowed to submit information and to deepen knowledge about Hashimoto's disease.
