Niniejsza praca zawiera podstawy teoretyczne i gotowy produkt - aplikację projektu Systemu Obsługi Podstawowej Opieki Zdrowotnej. Składa się ona ze wstępu dwóch rozdziałów, wniosków oraz opisu materiałów z jakich autor korzystał podczas pisania a także spis diagramów.We wstępie autor opisuje m.in. co to jest służba zdrowia, ośrodek, POZ, jak narodził się pomysł projektu oraz jaki jest cel powstania takiej aplikacji – systemu i jakie są oczekiwania względem niej. Rozdział I poświęcony jest rozważaniom teoretycznym jakimi są podstawy koncepcyjne, obszary aktywności, oczekiwane funkcje systemu z różnych punktów widzenia oraz typy dokumentów przetwarzanych w systemie. Najważniejszą częścią tego rozdziału są diagramy. Szczególnie ważnym jest diagram ERD w oparciu o który każdy programista z łatwością zorientuje się w strukturze danych. Rozdział II to opis wykonanego przykładowego systemu; rozdział ten może być wykorzystany jako zupełnie samodzielna instrukcja obsługi systemu ponieważ zawiera dokładny opis poszczególnych funkcji systemu wyszczególniających jak krok po kroku wykonywać określone zadania. Materiał podzielony jest na podpunkty jako logiczne części odpowiadające poszczególnym opcjom w programie.Wnioski opisują osiągnięte cele oraz możliwości i plany dalszej rozbudowy systemu.Na końcu pracy znajdują się spisy: bibliografii w postaci materiałów drukowanych oraz dostępnych w sieci internet a także spis diagramów. W niniejszej pracy – ze względów objętościowych - nie został ujęty kod programistyczny oraz wzorcowe pliki specyfikacji. Zarówno kod jak i pliki zostały dołączone do wersji elektronicznej.

The present work includes the theoretical basis and the finished product - Application Service System Project Primary Health Care. It consists of an introduction, two chapters, conclusions and a description of the materials from which the author used when writing and a list of diagrams.In the introduction author describesamong others what is a health resort, POZ, how arose the application’s idea, what is the purpose of the creation of applications - system and what are the expectations for it.Section I is dedicated to theoretical considerations which are the conceptual basis, the areas of activity, the expected functions of the system from different points of view and types of documents processed in the system. The most important part of this chapter are diagrams. Especially important is the ERD diagram on the basis of which any developer can easily recognize in the data structure.Section II is a description of an exemplary system made; This section can be used as a completely independent system manual as it contains a precise description of the various functions of the system detailing step by step how to perform certain tasks. The material is divided into subsections as logical parts corresponding to different options in the program. Conclusions describe the objectives achieved and the opportunities and plans for further development of the system. At the end of the work are the lists: the bibliography in the form of printed materials and available on the Internet and the list of diagrams.In this work - the reason of space - has not been included programming code and standard specification files. Both the code and the files are attached to the electronic version.
