| abstract in Polish: | Cel pracy: prospektywne porównanie wpływu przezskórnej i doustnej podaży 17β-estradiolu oraz rodzaju zastosowanego gestagenu na stężenia glukozy i insuliny u kobiet po menopauzie. Materiał i metody: Badaniami objęto 62 kobiety po menopauzie, dotychczas niestosujące HTZ, u których rozpoczęto suplementację wg następujących schematów: grupa A - 25 pacjentek, przezskórna ciągła suplementacja 17β-estradiolem w dawce 0,05 mg/24 godz. (Oesclim 50 - Fournier-Solvay) z doustną podażą gestagenu - dydrogesteronu w dawce 5 mg/24 godz. (Duphaston - Solvay), grupa B - 10 pacjentek, doustnie 2 mg 17β-estradiolu oraz 10 mg dydrogesteronu (Femoston-Solvay), grupa C - 17 pacjentek przezskórna ciągła suplementacja 17β-estradiolem w dawce 0,05 mg/24 godz. (Oesclim 50 - Fournier-Solvay) z doustną podażą gestagenu - medroksyprogesteronu w dawce 5 mg/24 godz. (Gestomikron - Adamed). Grupę kontrolną (grupa D) stanowiło 10 pacjentek bez leczenia. Badania kliniczne i laboratoryjne wykonano u każdej z kobiet przed rozpoczęciem badania w 6. i 12. mies. jego trwania. W warunkach podstawowych oznaczono stężenie FSH, estradiolu, insuliny i glukozy. W trakcie doustnego testu obciążenia glukozą (OGTT) oznaczano stężenie glukozy oraz insuliny. Wyniki: W grupie A 12 mies. leczenia obniżyło średnie stężenie glukozy w 60. min OGTT. W grupie B 12 mies. leczenia obniżyło średnie stężenie glukozy na czczo. W grupie C średnie stężenie insuliny na czczo oraz w 60. min po obciążeniu było w 12. mies. leczenia wyższe niż w chwili rozpoczęcia badania. W grupie kontrolnej nie obserwowano zmian średnich stężeń glukozy i insuliny w surowicy. Wniosek: Rodzaj zastosowanego gestagenu w hormonalnej terapii zastępczej w większym stopniu niż droga podaży estradiolu może mieć wpływ na zmiany metabolizmu insuliny i glukozy u kobiet po menopauzie. |
| abstract in English: | Aim: Comparison of the influence of transdermal and oral route of 17β-oestradiol administration and type of gestagen on plasma glucose and insulin concentration in postmenopausal women. Materials and methods: 62 women were enrolled in the study. Group A - 25 women received transdermal 17β-oestradiol (Oesclim 50 - Fournier-Solvay) combined with 5 mg oral dydrogesterone daily (Duphaston - Solvay). Group B - 10 women received oral 17β-oestradiol combined with 10 mg of dydrogesterone daily administration (Femoston-Solvay). Group C - 17 women received transdermal 17β-oestradiol (Oesclim 50 - Fournier-Solvay combined with 5 mg daily oral medroxyprogesterone (Gestomikron - Adamed). Group D - 10 women without treatment. Basic plasma FSH, oestradiol, glucose and insulin as well as glucose and insulin at the 60th minute of OGTT were measured initially and at the 6th and 12th month of the study. Results: The mean plasma glucose level during OGTT was reduced after 12 months of replacement therapy in group A. Mean plasma fasting glucose was reduced also after 12 months of oral replacement therapy in group B. The mean plasma fasting and glucose stimulated insulin concentrations were higher after 12 months of replacement therapy in group C. No changes in plasma glucose and insulin concentrations were observed in control group D. Conclusion: Administration of hormone replacement therapy may influence the metabolism of glucose and insulin in postmenopausal women according to the type of gestagen applied. |
