Rak piersi jest najczęściej występującym nowotworem złośliwym u kobiet w Polsce. Mimo ogromnego postępu w diagnozowaniu i leczeniu tego nowotworu, wciąż nie maleje umieralność nim spowodowana. Częstość zachorowań na raka piersi osiąga szczyt w wieku okołomenopauzalnym, aż 70% kobiet z rozpoznanym nowotworem piersi to osoby po 55 roku życia. Szansą na wyleczenie jest wykrycie raka we wczesnym stadium, gdy rozległość nowotworu jest mała i nie występują odlegle przerzuty. Najczęściej stosowaną metodą leczenia jest zabieg operacyjny – mastektomia , podczas którego zostaje odjęta pierś wraz okolicznymi węzłami chłonnymi. Zabieg ten jest rozległy i powoduje u kobiet wiele niekorzystnych skutków ubocznych, zarówno fizycznych jak i psychicznych. W zależności od stadium nowotworu oprócz operacji pacjentki są poddawane leczeniu hormonalnemu, radioterapii i chemioterapii. Choroba nowotworowa powoduje silne reakcje emocjonalne, jak lęk, niepokój i poczucie zagrożenia, dlatego leczenie raka piersi jest leczeniem ukierunkowanym nie tylko na trwałe wyleczenie pacjentki, lecz także przywrócenie sprawności fizycznej po zabiegu oraz zachowanie wsparcia psychicznego, prowadzonego w formie rehabilitacji fizycznej, psychicznej i społecznej. Opieka pielęgniarska nad pacjentką z nowotworem piersi po mastektomii jest opieką holistyczną, obejmującą rozwiazywanie problemów zdrowotnych, uświadamianie chorej odpowiedzialności za swoje zdrowie i motywowanie do zmiany stylu życia na prozdrowotny. Jednym z zadań pielęgniarki jest również współpracowanie z rodziną chorej, aby nauczyć ją opieki nad pacjentką, umocnić więzy i pozytywne nastawienie chorej, zmotywować chorą i jej rodzinę do podejmowania właściwych działań na rzecz własnego zdrowia.Praca składa się z pięciu zasadniczych części. Rozdział pierwszy jest oparty o literaturę przedmiotu i wyjaśnia wszystkie zagadnienia zawarte w tytule pracy. Zawiera informacje dotyczące budowy i funkcji gruczołu piersiowego, epidemiologię i czynniki ryzyka rozwoju nowotworu, objawy kliniczne raka piersi, metody leczenia, opiekę w okresie przed i pooperacyjnym, możliwe powikłania po zabiegu oraz opisuje jakość życia kobiet po zabiegu operacyjnym. Część druga to metodologia pracy, w której zaprezentowano cele pracy, problemy badawcze, metody, techniki i narzędzia badawcze oraz charakterystykę terenu i przebiegu badań. W kolejnym rozdziale zawarto analizę przypadku, wywiad z pacjentką, ocenę stanu biopsychospołecznego, przebieg choroby i leczenia na podstawie dokumentacji medycznej oraz opis zindywidualizowanej opieki pielęgniarskiej. Rozdział czwarty przedstawia model opieki nad pacjentką, zawierający najważniejsze problemy pielęgnacyjne w opiece pooperacyjnej oraz podkreśla rolę personelu pielęgniarskiego w opiece nad pacjentką po mastektomii. Całość jest zakończona podsumowaniem i wnioskami wynikającymi z napisanej pracy.
Breast cancer is the most common malignant neoplasm in women in Poland. Despite the enormous progress in the diagnosis and treatment of this cancer, the mortality caused by it is still not decreasing. The incidence of breast cancer reaches its peak in the perimenopausal age, as many as 70% of women diagnosed with breast cancer are over 55 years of age. The chance to be cured is to detect cancer at an early stage, when the tumor size is small and there are no distant metastases. The most common method of treatment is surgery - mastectomy, during which the breast and the surrounding lymph nodes are removed. This treatment is extensive and causes many negative side effects, both physical and mental, for women. Depending on the stage of the cancer, in addition to surgery, patients undergo hormonal treatment, radiotherapy and chemotherapy. Neoplastic disease causes strong emotional reactions, such as fear, anxiety and a sense of danger, therefore the treatment of breast cancer is a treatment aimed not only at permanently curing the patient, but also restoring physical fitness after the procedure and maintaining mental support in the form of physical, mental and social rehabilitation. Nursing care for a patient with breast cancer after mastectomy is a holistic care, including solving health problems, making patients aware of their responsibility for their health and motivating them to change their lifestyle to a healthy one. One of the tasks of a nurse is also to cooperate with the patient's family to teach her how to care for the patient, strengthen the patient's bonds and positive attitude, and motivate the patient and her family to take appropriate actions for their own health.The work consists of five main parts. The first chapter is based on the literature on the subject and explains all the issues contained in the title of the work. It contains information on the structure and function of the breast, epidemiology and risk factors for cancer development, clinical symptoms of breast cancer, treatment methods, care in the pre- and postoperative period, possible complications after surgery, and describes the quality of life of women after surgery. The second part is the methodology of work, in which the objectives of the work, research problems, methods, techniques and tools as well as the characteristics of the area and the course of research are presented. The next chapter contains a case study, an interview with the patient, an assessment of the biopsychosocial status, the course of the disease and treatment based on medical records, and a description of individualized nursing care. The fourth chapter presents the model of patient care, which includes the most important nursing problems in postoperative care and emphasizes the role of nursing staff in caring for a patient after mastectomy. The whole thing is finished with a summary and conclusions resulting from the written work.
