Model opieki pacjenta z przewlekłym zapaleniem trzustki uzależnionego od alkoholu

Przewlekłe zapalenie trzustki to schorzenie spowodowane długotrwałym procesem zapalnym z nieodwracalnym, postępującym zanikiem miąższu trzustki, włóknieniem narządu oraz narastającą niewydolnością zewnątrz i wewnątrzwydzielniczą. Patogeneza PZT nie została całkowicie poznana. Najczęstszą przyczyną PZT jest nadużywanie alkoholu. Zależnie od czynnika etiologicznego, przewlekłe zapalenie trzustki może mieć różny przebieg i obraz kliniczny. Głównymi dolegliwościami towarzyszącymi chorobie są silne bóle brzucha o różnym stopniu natężenia oraz objawy zewnątrz i wewnątrzwydzielniczej niewydolności trzustki. Rozpoznania dokonuje lekarz na podstawie zaobserwowanych w badaniach dodatkowych (USG, RTG, TK) charakterystycznych zmian w miąższu trzustki. Postawienie trafnej diagnozy w początkowej fazie choroby jest trudne, ponieważ we wczesnym stadium wyniki badań diagnostycznych nie ukazują żadnych odchyleń od normy. Przewlekłe zapalenie trzustki z towarzyszącą chorobą alkoholową jest przyczyną wielu problemów zdrowotnych. Schorzenia te wymagają długotrwałego leczenia, wiążą się z ryzykiem występowania dolegliwości bólowych i obniżeniem jakości życia pacjenta. Celem pracy jest opracowanie modelu opieki nad pacjentem z przewlekłym zapaleniem trzustki uzależnionym od alkoholu, postawienie diagnoz pielęgniarskich i zaproponowanie możliwych interwencji. Praca składa się z dwóch części. W pierwszej części pracy przedstawiono anatomię i fizjologię trzustki. Podano definicję choroby, jej patogenezę, epidemiologię, klasyfikację schorzenia, główne objawy, sposób diagnozowania i leczenia choroby oraz wpływ alkoholu na jej rozwój. Druga część pracy prezentuje model opieki nad pacjentem z przewlekłym zapaleniem trzustki uzależnionym od alkoholu, który został opracowany w oparciu o założenia teoretyczne modelu opieki pielęgniarskiej według Dorothei Orem. Najbardziej właściwa do zastosowania w tym przypadku jest druga składowa modelu Dorothei Orem czyli teoria deficytów samoopieki, która pozwala określić stopień w jakim choremu potrzebna jest pomoc pielęgniarki. W tej części pracy przedstawiono także 12 najczęstszych diagnoz pielęgniarskich. Praca została napisana na podstawie analizy dostępnej literatury i czasopism naukowych z dziedziny interny i gastroenterologii. Pracę zakończono podsumowaniem i przedstawieniem wynikających wniosków.
Chronic pancreatitis is a disease caused by prolonged inflammatory process with an irreversible, progressive loss of pancreatic parenchyma, fibrosis and organ failure growing outside and endocrine. The pathogenesis of CP is not completely understood. The CP is mostly caused by alcohol abuse. Depending on the etiological agent, the chronic pancreatitis may have a different course and clinical picture. The main symptoms associated with disease are severe abdominal pain of varying intensity and signs of extracellular and endocrine pancreatic insufficiency. The diagnosis is made by the clinician based on observed in additional studies (ultrasound, X-ray, CT) characteristic changes in the pancreas. Right diagnosis at the initial stage of the disease is difficult, because in the early stages of the results of diagnostic tests did not reveal any abnormalities. Chronic pancreatitis associated with alcoholism is the cause of many health problems. These diseases require long-term therapy, are associated with the risk of pain and decrease the quality of life of the patient.The aim of the thesis is to elaborate a model of care for patients with chronic pancreatitis, alcohol addiction, and to place nursing diagnoses and propose possible interventions. The thesis consists of two parts. The first part presents the anatomy and physiology of the pancreas. It contains the definition of the disease and its pathogenesis, epidemiology, the classification of the disease, the main symptoms, diagnosis and treatment of disease and the effects of alcohol on its development. The second part of the thesis presents a model of care for patients with chronic pancreatitis, addicted to alcohol, the model of care was developed based on the theoretical assumptions of the model of nursing care by Dorothea Orem. The last part is the theoretical assumptions of the model of nursing care by Dorothea Orem. This part of the thesis presents the 12 most common nursing diagnoses. The thesis was written based on the analysis of the available literature and scientific journals in the field of internal medicine and gastroenterology. The thesis has been completed with a summary and conclusions. The last part of the thesis is the list of references used to write it.
