Celem pracy była ocena funkcji nerek w doświadczalnym modelu krwotocznego zapalenia pęcherza moczowego (HC) wywołanego przez podawanie cyklofosfamidu (CP). W doświadczeniu wykorzystano szczury rasy Wistar (n=16), które zostały podzielone na dwie równe grupy badawcze: 1 (osobniki z modelem HC) oraz 2 (kontrolna). W grupie 1 wywołano HC poprzez czterokrotne dootrzewnowe podanie CP (75 mg/kg m.c.), natomiast osobniki z grupy 2 poddawane były pozorowanemu leczeniu roztworem 0,9 % NaCl. Na początku i na końcu trwania eksperymentu przeprowadzono dobową zbiórkę moczu celem ilościowej oceny diurezy oraz celem pobrania próbek moczu do następczej analizy laboratoryjnej. Następnie zwierzęta uśmiercono, pobrano im krew do analizy oraz pęcherz moczowy i nerki do oceny histopatologicznej. W grupie 1 wykazano statystycznie znamienne obniżenie dobowego wydalania z moczem Na i K, mocznika oraz kwasu moczowego, a także spadek frakcjonowanego wydalania sodu (FENa) oraz wskaźnika uszkodzenia nerek (RFI). W analizie histopatologicznej nerek nie wykazano żadnych zmian morfologicznych mogących być przyczyną obserwowanych zaburzeń funkcjonalnych nerek przy potwierdzeniu obecności zmian zapalnych w pęcherzu moczowym. Przy przyjętym schemacie dawkowania CP ujawniona czynnościowa dysfunkcja nerek miała zatem charakter ostrego przednerkowego uszkodzenia nerek (AKI). Biorąc pod uwagę również inne doniesienia literaturowe, należy stwierdzić iż nefrotoksyczność CP zależy od czasu ekspozycji na ten związek oraz wielkości podawanych dawek.

The aim of the study was to assess renal function in an experimental model of hemorrhagic cystitis (HC) evoked by cyclophosphamide (CP) in rats. The animals used in the experiment (n = 16) were divided into two equal groups: 1 (HC group) and 2 (control one). HC model was induced in animals constituting group 1 by four-times intraperitoneal injection of CP (75 mg / kg b.w.), while animals in group 2 were sham-treated with saline solution. Both at the beginning and at the end of the experiment, the daily urine output was measured to estimate 24-hour diuresis and to take urine samples for subsequent laboratory analysis. Finally, all animals were sacrificed, the blood samples were collected for further biochemical assessment and the nephrectomy and cystectomy were performed to obtained bladder and kidney tissues for histopathological evaluation. Statistically significant reduction of urinary excretion of Na and K, urea and uric acid, along with a drop in fractional excretion od sodium (FENa) and renal failure index (RFI) were demonstrated in group 1. The histopathological kidney assessment did not show any morphological changes that could be the reason for the observed kidney dysfunction while it confirmed the presence of inflammatory lesions inside the bladders. Therefore, it can be concluded that the revealed functional kidney damage meets the acute kidney injury (AKI) criteria. Moreover, taking into account the other reports concerning CP nephrotoxicity it should be emphasized that the discussed issue depends on the time of exposure to CP and the applied doses regimen.
