Postępy w terapii zakażeń HIV.

Human immunodeficiency virus (HIV) has been known to mankind since 25 years. Despite the wide presence of so many antiretroviral drugs on the pharmaceutical market, we still are not able to remove the virus effectively from the human body. However in the last decade a big progress took place in the field of HIV infection treatment.In the introductory section of the presented master’s thesis the history of HIV discovery and the newest epidemiological facts has been described. For better understanding of the presented drugs mechanism of action, the structure of virus, replication cycle and course of HIV infection has been discussed.In this study 11 new substances, introduced into treatment after a year 2000, have been characterized. Eight of them belong to known before therapeutic classes: emtricitabine – nucleoside reverse transcriptase inhibitor, etravirine – nonnucleoside reverse transcriptase inhibitor, lopinavir/ritonavir, fosamprenavir, atazanavir, darunavir, tipranavir – all protease inhibitors and tenofovir – nucleotide reverse transcriptase inhibitor (other substances from this class are known, but tenofovir is the first one which is used in HIV infection treatment). Three drugs from completely new classes, which mechanisms of action have not been known before were described: enfuvirtide – HIV fusion inhibitor, maraviroc – CCR5 receptor antagonist and raltegravir – integrase inhibitor.In the profile of every drug’s mechanism of action, mechanism of resistance, usage and efficacy, pharmacokinetic, interactions and side effects were included.
HIV jest wirusem znanym ludzkości od ponad 25 lat. Mimo obecnej na rynku farmaceutycznym dużej ilości leków antyretrowirusowych nadal nie potrafimy skutecznie wyeliminować go z organizmu człowieka. W ostatnim dziesięcioleciu dokonał się jednak duży postęp w dziedzinie terapii zakażenia HIV.We wstępie niniejszej pracy opisano krótko historię odkrycia wirusa oraz najnowsze dane epidemiologiczne. W celu lepszego zrozumienia mechanizmów działania leków omawianych w dalszej części pracy, scharakteryzowano także budowę wirusa oraz przebieg zakażenia.W niniejszym opracowaniu omówiono 11 nowych substancji wprowadzonych do lecznictwa po 2000 roku. Osiem z nich należy do znanych wcześniej grup terapeutycznych: emtrycytabina z grupy nukleozydowych inhibitorów odwrotnej transkryptazy, etrawiryna z grupy nienukleozydowych inhibitorów odwrotnej transkryptazy, lopinawir/rytonawir, fosamprenawir, atazanawir, darunawir i typranawir wszystkie z grupy inhibitorów proteazy oraz tenofowir z grupy nukleotydowych inhibitorów odwrotnej transkryptazy (znane są inne preparaty z tej grupy terapeutycznej, jednak tenofowir jest pierwszym wykorzystywanym w leczeniu zakażenia HIV). Omówiono również 3 leki z zupełnie nowych grup, o nieznanym wcześniej mechanizmie działania: enfuwirtyd z grupy inhibitorów fuzji, marawirok z grupy antagonistów koreceptora CCR5 oraz raltegrawir z grupy inhibitorów integrazy.W charakterystyce każdego leku uwzględniono jego mechanizm działania, mechanizmy oporności, zastosowanie i skuteczność, farmakokinetykę, interakcje oraz działania niepożądane.
